Policies and Fees

Insurance:  We are in Network with Tricare, TriWest, Cigna/Evernorth, Aetna, Optum and Magellan. 

If we are not credentialed with your specific insurance company, we will take cash payment at time of service and provide you with a superbill that you can send to your insurance company for potential reimbursement.  Any eligible reimbursement would then be paid directly from the insurance company to you.

Payment: For cash-pay clients, full payment for services is due at the time of service. Accepted methods include HSA or FSA cards, credit or debit cards, cash or checks. Insurance eligible clients must pay any co-pays, if applicable, at the time of service.  A $35 fee shall apply to all returned checks. We do not keep money onsite, so if paying by cash, please bring exact fee.

Cash-Pay Fees:
• Initial evaluation- $300 (60-90 min)
• Follow-ups - $150 (20-30 min)
• Extended appointment times pro-rated at $300 hourly rate
• No shows or late cancellations without 48-hour (business hours) notice: $100. Please note that no show fees are not covered by insurance, and this will be a cost to the individual patient, charged on the day of the no show.
• Patients expected to enroll in auto-pay for billing on day of service for co-pays, deductibles, non-covered costs.

Controlled Substances:  The provider has a conservative prescribing style, and if controlled substances were to be prescribed, a separate contract would need to be signed/agreed upon.

Scheduling:  
Prospective clients can call (858) 504-2308 and speak to RN Ronnie.  Patients would then receive invite to online patient portal and need to submit required information/documents at least 48 hours ahead of scheduled appointment. If information not completed 48 hours prior, appointment will be cancelled and need to be rescheduled.  Please be sure to include front/back of ID card, as well as insurance information as applicable. The intake questionnaire is lengthy but will allow Cindy to familiarize herself with your symptoms and goals ahead of the appointment and make best use of your scheduled time.
